For families of students with disabilities, ages 16-23, regarding IEP Transition Goals. Info will be shared with the VT Dept of Education to identify where things are working as well as where there may be gaps in the transition planning process. Be anonymous if you wish.
What are Transition Goals and Services? Transition services prepare students to make the transition from the world of school to the world of adulthood. The IEP team first considers vocational training, community employment and post secondary education for the student and, if appropriate, independent living and community participation also.
Transition services are a coordinated set of activities that are based on an individual transition assessment that identifies the student's needs, preferences and interests.surveymonkey.com/s/3N22ND3
